A copy of your pay slips for the last month, to verify your income

A copy of your partner's pay slips for the last month to verify their income

A copy of the most recent Centrelink payments received by either you or your spouse/partner and children to verify your receipt of Centrelink payments

A copy of letter from from a relevant health professional outlining your condition if you are not registered with Accessibility Services.

Accepted document formats are: jpg, jpeg, png, zip, doc, docx, pdf. Maximum document size is 2MB per file. If your document is over 2MB per file you will not be able to submit your application so zip the file. Also, your application will not be able to submit if the number of boxes you tick does not match the number of documents you attach. E.G if you tick 4 boxes but only attach 3 documents.
