This form should be used only by past students who are unable to log in and order online. To request a replacement testamur, apply here.

Processing information

Collect: You can collect your testamur in person at no cost. We will email you when your testamur is available for collection from Library and studentHQ (Hawthorn). Testamurs are held for 10 business days. See here for opening hours.

Mail: Postage and handling costs apply to receive your testamur by mail.

Domestic: $30.00
International: $85.00

You will be contacted for payment prior to your testamur being issued and posted.

Allow 4-6 weeks to process your request. Allow 7-10 business days after processing for delivery in Australia, longer for international.
