This form is only required to be completed by students, staff and contractors who attended a Victorian campus.

Swinburne will notify our community of COVID-19 cases and affected sites via the university’s COVID-19 hub using the information you provide in this form.

Under updated Department of Health requirements, you must notify social and workplace contacts, that you spent time with on-campus while infectious. Swinburne no longer identifies and informs your contacts individually, this is now the responsibility of the positive COVID-19 case.

Please remember to stay at home and isolate if you have tested positive for COVID-19 as per the advice provided to you by the Department of Health (DoH).
