What is a 'certified copy' of an original document?

I certify that the above information is true and correct. I understand that providing false or misleading information is in breach of University policy and constitutes a form of misconduct which may result in academic or other penalties.

The University is subject to government reporting obligations. Therefore if my gender or legal name (as applicable) is not changed with all government agencies with whom the University reports student data, making a change at the University may have unexpected negative consequences. An example is Centrelink payments of student support, these may be interrupted if the University records no longer match my record that exist with Centrelink. I acknowledge that in making this change I am aware of the possible impacts.

I acknowledge that any change in my residency or citizenship from that of an international student to a domestic student means that I cannot continue my application/ enrolment as an international student, and I request that the University amend my application/offer/enrolment (as applicable) to that of a domestic student. I acknowledge that any offer into a course as a domestic student is subject to places being available, and any applicable entry requirements.
