Flu Clinics 2011

Follow these easy steps.

1. Register with the Health Service

If you are a new patient and have not already registered with the Health Service please click here.

2. Complete the Consent Form

In order to have your flu vaccine, you need to download the consent form, complete and bring the form with you on the day of your immunisation. If you do not have the form, you will not be able to have the Flu vaccine on the day.

3. Cost

Cost of Flu vaccines for 2011: Staff - $22.00 and Students - $15.00. All Doctor consultations will be bulk billed for Flu vaccines.

4. Dates

Contact the Health Service on 9214 8483 to make an appointment. Don't forget to bring your consent form with you.
