Person centred wellness and reablement: Review a service specific goal directed care plan

|Pre-requisites||Direct involvement in the review of care plans, and a basic understanding of goal directed care planning|
Focus on what is required to work collaboratively with clients and/or their carers to review a service specific goal directed care plan.
- Use effective communication techniques to engage clients and/or their carers in a meaningful discussion to understand their progress towards goal achievement
- Review client outcomes (goals) as documented in the client’s Goal Directed Care Plan
- Document and report the outcomes of a review
This course is open to staff and volunteers of CHSP and/or HACC PYP providers in Victoria.
It is suitable for managers, team leaders, coordinators, supervisors, allied health and nursing professionals, and case managers.
This course is suitable for staff who are directly involved in reviewing client care plans and who want to build and/or refresh their understanding about how to review a client’s goals within a service specific Goal Directed Care Plan.
This course is not recommended for staff who are not directly involved in the review of care plans or who have not been involved in basic Goal Directed Care Planning training. These staff should instead consider Implementing and monitoring goal directed care plans in a service setting or Service specific care planning.
What to bring
Please bring along a notebook and pen to take your own notes. In addition, we’ll supply you with a workbook.
Receive a Victorian Community Care Education and Training Service (VCCETS) Certificate of Attendance upon completion of this course.
The certificate complements any existing qualifications you have working in the Commonwealth Home Support Programme (CHSP) and the Victorian Home and Community Care Program for Younger People (HACC PYP).
Statement of attainment
If you wish to be issued with a unit of competency statement of attainment, you will need to undertake further assessment. This assessment will cover both knowledge and skill components. You may be required to provide additional information about how you are applying your workshop training in the workplace.
