The following form is not necessary for direct Swinburne employees, who may use their Swinburne ID card to borrow. Use this form if you are employed via an agency or contractor.

An annual fee applies for alumni membership, to be paid when you pick up your access card. Alumni membership allows for borrowing of physical items but most electronic resources are excluded.
