Person centred wellness and reablement: Implementing and monitoring goal directed care plans in a service setting

Focus on what is required to work collaboratively with clients and/or their carers while supporting and monitoring goal achievement within an established Goal Directed Care Plan.
- Understand the value of documenting client goals in a Goal Directed Care Plan and how this is supported within the community care sector
- Describe and discuss the monitoring role of the direct care worker in supporting client goals within a wellness approach
- Identify and reflect on practices that incorporate a wellness approach, the person’s preferences and strengths and organisational policies and procedures
- Develop, document and/or report progress of a person’s care goals or care plan
This course is open to staff and volunteers of CHSP and/or HACC PYP providers in Victoria.
It is suitable for community care workers, direct care workers, allied health assistants, social support workers, and volunteers.
This course is suitable for all new or existing staff and volunteers who want to build and/or refresh their understanding about goal directed care planning, and their role in monitoring and supporting clients within the care planning process. This course is designed for staff not directly involved in the development of care plans. A basic understanding of the wellness approach is required.
This course is not recommended for staff who are directly involved in the development of care plans. These staff should instead consider Service specific care planning, Embedding goal directed care planning (advanced), or Review a service specific goal directed care plan.
What to bring
Please bring along a notebook and pen to take your own notes. In addition, we’ll supply you with a workbook.
Receive a Victorian Community Care Education and Training Service (VCCETS) Certificate of Attendance upon completion of this course.
The certificate complements any existing qualifications you have working in the Commonwealth Home Support Programme (CHSP) and the Victorian Home and Community Care Program for Younger People (HACC PYP).
Statement of attainment
If you wish to be issued with a unit of competency statement of attainment, you will need to undertake further assessment. This assessment will cover both knowledge and skill components. You may be required to provide additional information about how you are applying your workshop training in the workplace.
