Welcome to Disability Services Registration

Students who have a disability or medical condition must register with Disability Services in order to claim support.

Please follow the steps below for registration.

Step one: Download and print the documents

Download the documents below, complete the confidentiality form and have your relevant health professional complete the registration form.
  1. Eligibility Guidelines
  2. Service Guidelines and Agreement
  3. Disability Documentation Form
  4. Consent to Release Information Form

Step two: Complete the following tasks

Select the checkbox below if all your forms are ready.
  1. I have read the Eligibility guidelines. Please make a selection.
  2. I have read and signed the Service Guidelines and Agreement Please make a selection.
  3. I have completed my Disability Documentation Form with a relevant professional. Please make a selection.
  4. I have filled and signed the Consent to Release Information form. Please make a selection.

If you have ticked all the four options you can now proceed to register. Click next to continue.

Please Note:

All of your above documents need to be scanned and attached with the Registration form. If you don't have access to a scanner you can send the documents via post to:
