Swinburne Experience Survey - Workplace Delivery
To best meet the needs of our Enterprise delivery students, a more targetted version of the Swinburne Experience Survey has been created to be delivered on paper, or online. Paper forms can only be scanned if completed with a valid student ID.
There are a number of ways that this survey can be administered.
- Blank forms distributed by the teacher/trainer. Student ID and course details completed by the student and/or teacher/trainer, collected by Swinburne or workplace staff and returned to SPQ in Swinburne mail with a completed batch sheet..
- Pre-printed forms with student id and course details sent to the teacher/trainer or coordintor to be distributed to students and collected by Swinburne or workplace staff and returned to SPQ in Swinburne mail.
- Pre-printed forms with Student ID and course details in individual envelopes for each student which are sent to individual students workplace to be distributed to students and completed surveys sealed in reply paid envelopes (provided) and collected by Swinburne or workplace staff and returned to SPQ in Swinburne on large mail envelope
- Pre-printed forms with Student ID and course details in individual envelopes for each student including a reply paid mail envelope addressed to SPQ which are sent to individual students workplace or home address. Students complete and return to SPQ in reply paid envelope either individually or through their workplace.
To order the forms through SPQ, please download the following Excel spreadsheet choose your administration method and complete both the "Course Details" and the "Student Details" sheets. Once you have completed ONE FORM FOR EACH COURSE, save for your records and send a copy to Adele Drago in SPQ.

Schools have been sent a listing of all courses which have the campus listed as Enterprise broken down by department, and should choose courses which are currently being delivered. You can use these sheets to copy in Student IDs and timetable groups. If you have courses primarily delivered in the workplace which are not listed as Enterprise, you are still able to use this process if approved by the School or Department as appropriate.
The survey is best if administered as near the end of the course as possible, though valuable feedback could also be collected near the mid point of a course where it is delivered over a longer timeline.
