Review Request

What is the decision or outcome that you are applying to have reviewed? The outcome must be a Reviewable Decision as defined by the Review and Appeals Regulation.



Note: You must upload a copy of the decision or outcome you are seeking to be reviewed at the end of this online form. (Example: complaint outcome; academic progress review notice of decision; proposed cancelation of enrolment letter)

Note: an application for review must be received within 21 working days of being notified of the decision.

Please provide an explanation for your late application (your application will be considered late if it has been lodged more than 21 working days after the date of your original notification). If the application is late, it will be rejected unless the Academic Registrar decides to accept it as a late application.

State your case for review. Please specify the grounds upon which you wish to have the decision reviewed, explain your reasons concisely, present any further evidence you believe should be considered, and set out the outcome you desire. Mandatory
