Timetable Activity Amendment Forms

The Timetable Office is responsible for timetabling all Units of Study for Higher Education into Syllabus Plus. When the timetable is published on the Web, appropriate forms are used as part of the Policy and Procedure Documents. These forms are available below with a brief description explaining the purpose for each form:

Types of Amendment Forms Description
 
Published Timetable Change Request Form This form is used for changes to activity requests to units. Faculties are required complete and submit this form 6 days prior Allocate+ open. The form requires approval by the Dean or the Deputy Dean of the Faculty requesting the change.
 
Published Timetable Change Multiple Request Form This form is used for changes to multiple activity requests to units. Faculties are required complete and submit this form 6 days prior to Allocate+ open. The form requires approval by the Dean or the Deputy Dean of the Faculty requesting the change.
 
Request to Amend Final Timetable Activity Form This form is used for changes to activity requests to units. Faculties are required complete and submit this form 5 days prior to Allocate+ open and once Allocate+ is open. The form requires approval by the Dean or the Deputy Dean of the Faculty requesting the change. The form is sent to the Timetable Office and forwarded to the DVC (Academic).
 
Request to Teach Outside of Standard Teaching Period Form As outlined in the Unit of Study Delivery and Timetabling - Higher Education Policy, any variations to that policy require approval by the Deputy Vice-Chancellor (Academic) prior to timetabling or HESA reporting. Faculties are required to submit this completed form to the Timetable office. The form requires approval by the Dean or the Deputy Dean of the Faculty requesting the change. The form is forwarded to the DVC (Academic) by the Timetable office.
